Free → https://pilotdebrief.com/pattern On a summer afternoon in July 2024, an Air Tractor AT-802A Fire Boss dropped toward a choppy Montana lake to scoop water for a wildfire fight — and drifted just far enough left to meet the rock face along the shoreline. The pilot was flying the No. 2 position in a four-ship air tanker formation supporting the Horse Gulch Fire near Helena. After a nearly two-hour transit from their home base, the group arrived on scene and moved to nearby Hauser Lake to begin water-scooping runs. The accident happened on the very first scoop sequence of the mission. The lead airplane completed its scoop cleanly and climbed away. As the float-equipped accident airplane descended toward the choppy surface, the lead pilot radioed an instruction to abort the scoop because of a tailwind. That call was never acknowledged, and no evasive action followed. Witnesses watched the airplane move unsteadily side to side before the left wing dipped into the water, the right wing rose, and the airplane veered left into a vertical rock face along the shoreline. Flight tracking data captured the drift: over the final 1,447 feet of the run, the airplane wandered roughly 10 degrees left, away from the middle of the waterway and toward the shore — a drift that lined up in time with the lead airplane's own turn. The single occupant did not survive. This was the pilot's first fire season, and training records showed a documented pattern: during tactical flying, situational awareness would slip, fundamental skills would regress, and the pilot would freeze at the controls. Investigators concluded the accident traced to a loss of situational awareness driven by task overload in the demanding multi-airplane firefighting environment, which left the pilot unable to maintain directional control before the collision with terrain. It's the pattern that recurs whenever workload outpaces experience: the airplane was flyable, the mechanics were sound, and the margin that vanished was human bandwidth.
