The ride, which had operated since 1986, malfunctioned after a water pump failed, causing a raft to become stranded on a metal support rail. A second raft collided with it and was pulled into the ride’s conveyor mechanism, flipping and crushing four adults, while two children survived. Investigations revealed longstanding safety issues, including previous similar incidents, a lack of automatic emergency shutdown systems, and inadequate staff training. Dreamworld’s parent company, Ardent Leisure, was fined $3.6 million and paid additional settlements to victims’ families and first responders. The ride was permanently closed and buried under concrete.
